INSTRUCTIONS
Please read Who Can Nominate; Nominee Eligibility; and, Levels of Recognition Criteria PRIOR to completing a Nomination Form. Additionally, for the categories of Lifetime Contribution and Community Pillars, a photograph of the nominee is required along with a letter of support.
Please read and complete the nomination form carefully and attach any requirements (photo, support letter(s), media samples) at the same time the nomination form is submitted. A nomination form may be saved so that you may return and amend and/or add any further information and/or attachments before submission.
Most importantly, this is not a competition but entirely merit-based. There are no deadlines and nominations are accepted for review when received.
